feat(dry-run): step numbering + --no-ansi flag#5546

When --debug, --steps, or --verbose is passed, prefix each leaf step with a per-test counter so the output is a clean numbered list (handy for AI consumption). MetaSteps stay unprefixed. Adds --no-ansi to disable colored / ANSI-styled output by setting chalk.level = 0. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>
