feat(dry-run): step numbering + --no-ansi flag

• Numbers each leaf step per test in dry-run output (when --debug/--steps/--verbose is on) so AI tools (and humans) can refer to a step by index. MetaSteps like Within \"...\" stay unprefixed.
• Adds --no-ansi flag to disable colored / ANSI-styled output (sets chalk.level = 0), useful when piping dry-run output to a file or feeding it to an LLM.

• dry-run --debug --grep within shows numbered steps, counter resets per test
